TRANSPORTATION
INFORMATION
I will be utilizing the transportation services of The Arc
of Baltimore. I understand that this service is a privilege
and is not a guaranteed service. I agree to abide by the
following provisions. If I fail to follow this agreement
transportation services will be terminated and other means
of transportation will need to be secured by myself, my
family or care giver.

My caregiver or I agree to contact my driver or their
supervisor when I am sick or unable to attend, as soon
as possible.

My caregiver or I agree to notify my driver or their supervisor
of planned vacations a week in advance.
- I
agree to be ready when the van arrives. Vans will wait
no longer than three minutes for individuals to get on
the van. Drivers will not knock on doors or blow horns
upon arrival. So, please be ready on time.
- Van
drivers are not permitted to pick up and deliver paychecks
if the employee is absent from work.

Van drivers are not responsible or knowledgeable about
events that happen on job sites and are not able to answer
questions relating to what happens at work.

I agree to follow all safety rules. Jeopardizing the safety
of myself, the driver or other passengers will result
in an immediate loss of van privileges.

agree to behave in a manner that does not interfere with
the well being of the other passengers.

agree to keep my driver informed of changes in my address
and emergency information as they occur.

If drivers are out sick or on emergency leave, attempts
will be made to find a substitute driver, however this
is not always possible. A back up transportation plan
should be developed by families/caregivers for these emergency
situations and it is the responsibility of the family/caregiver
to notify the job coach of the arrangements.

I am responsible for finding out holiday schedules from
my supervisor or job coach. Drivers may not be aware of
each individual job sites schedule. If I work at a supported
employment, medical day or day site I can call Fran Thomas
and if I work in Landscape or Janitorial I can call Laura
Wink. Both can be reached after 8:30 am for information.

In the event of inclement weather, your van driver will
make ever attempt to call you, however, if needed please
call your driver to find out if transportation is available.
Decisions on whether a van operates are based on the road
conditions in the area where we pick up individuals and
the route. Therefore some vans may operate and others
may not, so please call the individual driver. They will
be the first person who can answer your question.

If a driver is involved in a minor accident or experiences
a mechanical breakdown while in route to the center or
site, the driver will notify his/her supervisor and alternative
arrangements will be made to make sure all individuals
are safely transported in a back up vehicle.
- If
the driver is involved in an accident and individuals
are deemed to need medical attention, care givers will
be notified as soon as possible. Directors, Managers and
Employment Support Associates will make every effort to
meet the person at the hospital as needed.
- We
make every effort to be on time, however be please be
aware that times are effected by traffic, weather and
the time it takes individuals to get on and off the van
safely.
