- The
first step to applying for services is to complete an
application for services through the Developmental Disabilities
Administration (DDA).
-
DDA will review the application and assign it to a case
manager (or Service Coordinator) for eligibility determination.
Eligibility determinations are made through a face-to-face
interview with the applicant. DDA will assess applicants’
eligibility to receive services (meaning that the applicant
has a developmental disability-physical or mental impairment,
other than the sole diagnosis of a mental illness, it
manifested prior to the age of 22, and is expected to
continue indefinitely) and a level of priority status
.
-
Once DDA has determined that the applicant is eligible
to receive services and a priority status, the applicant
is eligible to receive Rolling Access funds and possibly
on-going funds for day, residential, or support services.
An applicant’s ability to access funding is based
both on their eligibility and availability of funds.
